{"id":31813,"date":"2020-02-28T00:00:00","date_gmt":"2020-02-28T00:00:00","guid":{"rendered":"http:\/\/medgoo.com\/index.php\/2020\/02\/28\/assisting-medication-continuity-cuts-readmissions-for-seniors\/"},"modified":"2020-03-02T17:10:13","modified_gmt":"2020-03-02T17:10:13","slug":"assisting-medication-continuity-cuts-readmissions-for-seniors","status":"publish","type":"post","link":"https:\/\/medgoo.com\/index.php\/2020\/02\/28\/assisting-medication-continuity-cuts-readmissions-for-seniors\/","title":{"rendered":"Assisting Medication Continuity Cuts Readmissions for Seniors"},"content":{"rendered":"<h3>Interventions that bridge care transitions most likely to impact medication continuity in older adults<\/h3>\n<p><b><\/b><\/p>\n<p><b><\/b><\/p>\n<p>FRIDAY, Feb. 28, 2020 (HealthDay News) &#8212; Interventions that bridge care transitions have the greatest impact on older adults&#8217; medication continuity, according to a review published online Feb. 20 in <i>Age and Ageing<\/i>.<\/p>\n<p>Justine Tomlinson, from the University of Bradford in the United Kingdom, and colleagues conducted a systematic literature review to identify randomized controlled trials evaluating interventions delivered during a hospital stay or after a recent discharge to support medication continuity in older patients.<\/p>\n<p>Based on 24 identified studies (with 17,664 participants), the researchers found that interventions that bridged the transition for up to 90 days were more likely to support successful transitions. Self-management activities (relative risk [RR], 0.81), telephone follow-up (RR, 0.84), and medication reconciliation (RR, 0.88) were statistically associated with reduced hospital readmissions in the meta-analysis.<\/p>\n<p>&#8220;Our results suggest that interventions that bridge the care transition best support older patients&#8217; medication continuity and have the greatest impact on reducing hospital readmission,&#8221; the authors write.<\/p>\n<p><a href=\"https:\/\/academic.oup.com\/ageing\/advance-article\/doi\/10.1093\/ageing\/afaa002\/5733075\" target=\"_new\" rel=\"noopener noreferrer\">Abstract\/Full Text<\/a><\/p>\n<p><i><\/i><\/p>\n<p><i>Copyright \u00a9 2020 <a href=\"https:\/\/www.healthday.com\/\" target=\"_new\" rel=\"noopener noreferrer\">HealthDay<\/a>.
