Dual-Biomarker Algorithm Can Inform Osteoarthritis Diagnosis

High accuracy seen for cartilage oligomeric matrix protein concentration and the cartilage oligomeric matrix protein to interleukin‐8 ratio

THURSDAY, Dec. 19, 2024 (HealthDay News) — A new diagnostic test using a dual-biomarker algorithm can inform osteoarthritis diagnosis with high accuracy, according to a study published online Dec. 18 in the Journal of Orthopaedic Research.

Daniel Keter, from CD Diagnostics in Claymont, Delaware, and colleagues developed a combinatorial algorithm using cartilage oligomeric matrix protein and interleukin-8 (IL-8) concentrations to differentiate primary osteoarthritis from inflammatory arthritis. A total of 171 human knee synovial fluid specimens were used to establish and validate clinical decision limits for cartilage oligomeric matrix protein concentration and the cartilage oligomeric matrix protein-to-IL-8 ratio.

The researchers found that clinical sensitivity and specificity were 87.0 and 88.9 percent, respectively, for the osteoarthritis algorithm, for overall accuracy of 88.3 percent. The osteoarthritis algorithm had an area under the curve of 0.93 in analysis of the receiver operating characteristic curve derived for the osteoarthritis algorithm ratios calculated on the primary osteoarthritis cohort versus the inflammatory arthritis cohort.

“Looking forward, differentiating primary osteoarthritis from other inflammatory arthritis types contributes towards better diagnosis that enables accurate, and targeted treatment of primary osteoarthritis,” the authors write.

All authors are paid employees of CD Diagnostics or CD Laboratories, which are subsidiaries of Zimmer Biomet. Several authors disclosed financial ties to Zimmer Biomet, which funded the study.
